Cancellation Policy

1. Cancellation, Rescheduling, Refund, and No-Show Policy • Appointments must be canceled at least 24 hours prior to a scheduled PT session as a courtesy. • We do not charge any fee for No-Shows. • There is no fee to reschedule and cancel the session prior to a scheduled PT session. • Late arrivals will not be provided additional time. If the patient wants additional time, we offer extension at $25/per additional 15 minutes (depending on PT's availability). • Payments are non-refundable once the PT session has started. • Refunds will not be issued for the completed sessions. • We reserve the right to discontinue our services to any patient for repeated cancellations, rescheduling requests, or non-compliance with our scheduling policies. 2. Financial Agreement • This practice is cash-based and does not accept any health insurance. We are an out-of-network provider. Payment is due at the time of service. • Currently, we do not accept any health insurance, including Medicare and Medicaid. As of now, we cannot treat the patients with Medicare. If your insurance plan provides an out-of-network benefits, you can request the reimbursement from your insurance company for our services. However, we do not guarantee the reimbursement from your health insurance. • You understand that no claims will be submitted to the health insurance on your behalf, and it is your responsibility to check whether your insurance provider offers reimbursement for out-of-network physical therapy services, including telehealth. • You acknowledge and agree that you will not attempt to submit claims to Medicare, as we are not Medicare providers.
